Woolly Hair with Systematized Epidermal Nevus. - Indian journal of dermatology
Woolly hair is a hair shaft disorder characterized by fine and tightly curled hair. Woolly hair could be syndromic (associated with systemic disease) or non-syndromic (not associated with any systemic disease). Woolly hair is described in association with many skin, dental, ophthalmic and cardiac anomalies but association of woolly hair with bilateral systematized epidermal nevus described in our case is first of its kind.
Impact of air pollutants from surface transport sources on human health: A modeling and epidemiological approach. - Environment international
This study adopted an integrated 'source-to-receptor' assessment paradigm in order to determine the effects of emissions from passenger transport on urban air quality and human health in the megacity, Delhi. The emission modeling was carried out for the base year 2007 and three alternate (ALT) policy scenarios along with a business as usual (BAU) scenario for the year 2021. An Activity-Structure-Emission Factor (ASF) framework was adapted for emission modeling, followed by a grid-wise air quality assessment using AERMOD and a health impact assessment using an epidemiological approach. It was observed that a 2021-ALT-III scenario resulted in a maximum concentration reduction of ~24%, ~42% and ~58% for carbon monoxide (CO), nitrogen dioxide (NO2) and particulate matter (PM), respectively, compared to a 2021-BAU scenario. Further, it results in significant reductions in respiratory and cardiovascular mortality, morbidity and Disability Adjusted Life Years (DALY) by 41% and 58% on exposure to PM2.5 and NO2 concentrations when compared to the 2021-BAU scenario, respectively. In other words, a mix of proposed policy interventions namely the full-phased introduction of the Integrated Mass Transit System, fixed bus speed, stringent vehicle emission norms and a hike in parking fees for private vehicles would help in strengthening the capability of passenger transport to cater to a growing transport demand with a minimum health burden in the Delhi region. Further, the study estimated that the transport of goods would be responsible for ~5.5% additional VKT in the 2021-BAU scenario; however, it will contribute ~49% and ~55% additional NO2 and PM2.5 concentrations, respectively, in the Delhi region. Implementation of diesel particulate filters for goods vehicles in the 2021-ALT-IV-O scenario would help in the reduction of ~87% of PM2.5 concentration, compared to the 2021-BAU scenario; translating into a gain of 1267 and 505 DALY per million people from exposure to PM2.5 and NO2 concentrations, respectively. These findings suggest that significant health benefits are possible if goods transport is also included while designing strategies and policies in order to improve the overall urban air quality and minimize health impacts in city areas.Copyright Â© 2015 Elsevier Ltd. Lymphoepithelioma-like carcinoma of the skin: an exceedingly rare primary skin tumor. - Indian journal of dermatology
Primary lymphoepithelioma-like carcinoma of the skin (LE-lCS) is a very rare cutaneous malignancy of uncertain origin. The neoplasm reveals typical morphological similarity to undifferentiated nasopharyngeal carcinoma (lymphoepithelioma). This case report presents a 47-year-old man with a 5 mm erythematous papule on dorsal nose of six months duration. The patient underwent complete surgical excision and is disease free 7 months later.
Estimation of organic and elemental carbon emitted from wood burning in traditional and improved cookstoves using controlled cooking test. - Environmental science & technology
Emission of various climate- and health-related pollutant species from solid biomass burning in traditional cookstoves is a global concern. Improved cookstoves serve as a possible solution to mitigate the associated impacts. However, there is a need to intensify the efforts in order to increase the data availability and promote revision of existing metrics of cookstove testing. In this study, the effect of different phases of a cooking cycle of Northern India on emission factors of OC and EC (char and soot) was assessed for four cookstoves (advanced, improved, and traditional) using Acacia nilotica. Lowest EFs for OC (0.04 g/MJ) and EC (0.02 g/MJ) were observed in case of the forced draft cookstove; while the traditional and natural draft top feed cookstove emitted the highest OC (0.07 g/MJ) and EC (0.09 g/MJ), respectively. Variation in terms of EFs for OC and EC (char and soot) within the cooking cycle was also found to be significant.
Real-time ultrasound guidance facilitates transradial access: RAUST (Radial Artery access with Ultrasound Trial). - JACC. Cardiovascular interventions
This study sought to assess the utility of ultrasound (US) guidance for transradial arterial access.US guidance has been demonstrated to facilitate vascular access, but has not been tested in a multicenter randomized fashion for transradial cardiac catheterization.We conducted a prospective multicenter randomized controlled trial of 698 patients undergoing transradial cardiac catheterization. Patients were randomized to needle insertion with either palpation or real-time US guidance (351 palpation, 347 US). Primary endpoints were the number of forward attempts required for access, first-pass success rate, and time to access.The number of attempts was reduced with US guidance [mean: 1.65 Â± 1.2 vs. 3.05 Â± 3.4, p < 0.0001; median: 1 (interquartile range [IQR]: 1 to 2) vs. 2 (1 to 3), p < 0.0001] and the first-pass success rate improved (64.8% vs. 43.9%, p < 0.0001). The time to access was reduced (88 Â± 78 s vs. 108 Â± 112 s, p = 0.006; median: 64 [IQR: 45 to 94] s vs. 74 [IQR: 49 to 120] s, p = 0.01). Ten patients in the control group required crossover to US guidance after 5 min of failed palpation attempts with 8 of 10 (80%) having successful sheath insertion with US. The number of difficult access procedures was decreased with US guidance (2.4% vs. 18.6% for â‰¥5 attempts, p < 0.001; 3.7% vs. 6.8% for â‰¥5min, p = 0.07). No significant differences were observed in the rate of operator-reported spasm, patient pain scores following the procedure, or bleeding complications.Ultrasound guidance improves the success and efficiency of radial artery cannulation in patients presenting for transradial catheterization. Design and characterization of controlled release tablet of metoprolol. - Journal of pharmacy & bioallied sciences
Metoprolol succinate is a selective beta-adrenergic receptor blocker useful in treatment of hypertension, angina and heart failure. The purpose of the present work was to design and evaluate controlled release matrix type tablet of Metoprolo succinate using HPMC K15M and Eudragit (RLPO and RSPO) as a matrix forming agents. Effect of various polymer alone and combinations were studied in pH 1.2 buffer using USP type II paddle at 50 rpm. HPMC was used to form firm gel with Eudragit polymer. Formulation with Equal proportion (1:1) of Eudragit RSPO and RLPO showed optimum drug release t(50)=7 hrs and t(100)=16 hrs indicate optimum permeability for drug release from matrix. The drug release mechanism was predominantly found to be Non-Fickian diffusion controlled.
Design and characterization of controlled release gastro-retentive floating tablet of an atypical psychotropic agent. - Journal of pharmacy & bioallied sciences
The purpose of the present work was to design and evaluate the once daily sustained release matrix type gastro-retentive floating tablet of Quetiapine Fumarate base on hydrophilic matrices of HPMC, sodium CMC and Carbopol. Sodium bicarbonate was incorporated as a gas-generating agent to give buoyancy. In-vitro drug release studies were performed in pH 1.2 buffer using USP type II paddle at 50 rpm. The release rate of drug decreased with increasing polymer proportion of HPMC K15M from 20 to 60 mg. Formulation with desired drug release achieved with combination of sodium CMC and K15M in ratio of 1:3. The drug release mechanism was predominantly found to be Non-Fickian diffusion and Higuchi controlled.
Release kinetics of Î²-cyfluthrin from its encapsulated formulations in water. - Journal of environmental science and health. Part. B, Pesticides, food contaminants, and agricultural wastes
Controlled release formulations of Î²-cyfluthrin, a non-systemic, broad spectrum contact insecticide, have been prepared using laboratory synthesized poly(ethylene glycol) (PEG) based amphiphilic copolymers. Copolymers of polyethylene glycols of different molecular weights and various dimethyl esters, viz. dimethyl isophthalate, which self assemble into nano micellar aggregates in aqueous media, have been synthesized. The kinetics of Î²-cyfluthrin from developed controlled release (CR) formulations were studied in comparison with that of the commercially available 025 SC. Release from the commercial formulation was faster than with the developed CR formulations. The rate of release of encapsulated Î²-cyfluthrin from nano micellar aggregates is reduced by increasing the molecular weight of PEG. The diffusion exponent (n value) of Î²-cyfluthrin in water ranged from 0.427 to 0.622 in the tested formulations. The release was diffusion controlled with a half-release time (t(Â½)) of 3.92 to 7.9 days in water from different formulations, and the period of optimum availability (POA) of Î²-cyfluthrin ranged from 1.4 to 20.5 days. The results suggest that the application rate of Î²-cyfluthrin can be optimized to achieve insect control at the desired level and period.
Construction of fuzzy membership functions for urban vehicular exhaust emissions modeling. - Environmental monitoring and assessment
This paper presents a method for constructing a membership function (MF) for the fuzzy sets that expert systems deal with. This paper introduces a Bezier curve-based mechanism for constructing MFs of convex normal fuzzy sets. The mechanism can fit any given data set with a minimum level of discrepancy. In the absence of data, the mechanism can be intuitively manipulated by the user to construct MFs with the desired shape. MFs have been developed using the proposed mechanism for urban vehicular exhaust emission modeling. It has been observed that all meteorological and vehicular parameters have either S-shaped MFs or Z-shaped MFs. Gaussian MF has been mostly applied for modeling air quality. The present study explored the application of fuzzy MF to analyze air pollution data from vehicular emission. The study reveals that S-shaped and Z-shaped MF can be used in addition to Gaussian MF.
Ten mg dexrabeprazole daily is as effective as 20 mg dexrabeprazole daily. - World journal of gastroenterology
Ten mg dexrabeprazole daily has been shown to be more effective than 20 mg rabeprazole daily against gastroesophageal reflux disease (GERD). This report shows that the efficacy of 10 mg dexrabeprazole daily is equivalent to that of 20 mg dexrabeprazole daily against GERD. This implies that a dose of 10 mg dexrabeprazole is sufficient to block the maximum amount of proton pumps without any need to double the dose as suggested with rabeprazole.
