Imaging features of bariatric surgery and its complications. - Seminars in ultrasound, CT, and MR
Diagnostic imaging features of the most commonly performed types of bariatric surgery, which are gastric banding, sleeve gastrectomy, and Roux-en-Y gastric bypass, are reviewed as well as imaging diagnosis of their complications. Although upper gastrointestinal series remains the first-line imaging test for assessing postoperative anatomy and complications, the important role of multidetector computed tomography in diagnosis of serious complications is highlighted.Copyright © 2013 Elsevier Inc. Lessons learned from quality assurance: errors in the diagnosis of acute cholecystitis on ultrasound and CT. - AJR. American journal of roentgenology
The purpose of this article is to study errors in the diagnosis of acute cholecystitis reported in the online departmental quality assurance (QA) database.The departmental QA database was searched from October 2005 to April 2010 for cases of acute cholecystitis. Errors were classified into overcalls and undercalls.We identified 14 cases of misdiagnosis involving acute cholecystitis. Three cases were classified as overcalls (21%) and eleven as undercalls (79%). Eight cases of misdiagnosis involved ultrasound studies (57%) and six cases involved CT studies (43%). Cases of overcall on ultrasound showed gallbladder wall edema, but none portrayed distention of the gallbladder. The final diagnosis in these cases included hepatitis, sepsis, and a case of chronic cholecystitis. All misinterpretations of CT cases were classified as undercalls. Contributing factors to misdiagnosis were lack of recognition of wall edema (n = 6), gallbladder distention (n = 4), absence of gallbladder wall edema (n = 1), lack of conclusion in the report (n = 2), and hospitalization in the ICU (n = 2). A possible case clustering was observed just after July almost every year.An important pitfall in the diagnosis of acute cholecystitis is lack of recognition of gallbladder wall edema on CT. A relaxed (nondistended) gallbladder provides important evidence against the diagnosis of acute cholecystitis. Intensive care patients with sepsis often have no specific signs for diagnosis of acute cholecystitis, making diagnosis especially challenging.
Multidetector CT urography: comparison of two different scanning protocols for improved visualization of the urinary tract. - Journal of computer assisted tomography
The goal of this study was to evaluate different CT scanning protocols on the depiction of the urinary tract by multidetector CT. The authors retrospectively reviewed 55 consecutive patients who underwent CT scanograms. Two groups of patients were included: renal donors (n=29) and hematuria patients (n=26). For the renal donor protocol, 120 mL of iodinated contrast was injected and a CT scanogram was obtained after a 5-minute delay. For the hematuria CT urography protocol, 100 mL of contrast was followed by a 250 mL normal saline drip and CT scanograms acquired after an 8-minute delay. Urinary tracts from both imaging protocols were then divided into four segments and evaluated by consensus reading of two experienced radiologists rated on a scale of 0 to 2. Complete visualization of the renal pelvis and the proximal, middle, and distal ureter for the renal donor protocol was noted to be 86%, 57%, 45%, and 52% and for that of the CT urography protocol to be 75%, 65%, 40%, and 44%, respectively. Comparing scanograms of the renal donor protocol and the CT urography protocol, there was no statistically significant difference in the depiction of renal pelvis or the proximal, middle or distal ureteral region (P=0.1625, 0.3226, 0.8636, and 0.6145, respectively). The study demonstrates that there is no significant difference between the CT urography protocol and the renal donor protocol in the depiction of the urinary tract.
Transpulmonary CT-guided radiofrequency ablation of liver metastasis. - Cardiovascular and interventional radiology
The percutaneous approach for RF ablation of liver lesions high up in the dome is always difficult. The authors describe a transpulmonary approach that may be simpler and safer to use.
